IDR Healthcare is seeking a travel nurse RN Home Health Care Manager for a travel nursing job in Charlottesville, Virginia.

Position: Registered Nurse (RN) – Case Manager (Home Health)
Location: Charlottesville, VA (coverage from Charlottesville to Staunton)
Start Date: ASAP
Contract Type: Contract
Schedule: Monday–Friday, flexible daytime hours
On-Call: Required one week per month (light triage-based call)

Join a dedicated home health team serving patients across Charlottesville and surrounding regions. This position is ideal for a self-directed RN Case Manager with strong experience in OASIS documentation, HCHB usage, and care coordination.

? Responsibilities & Role Overview

• Complete 6 points per day through a combination of admissions, revisits, recertifications, and discharges
• Perform comprehensive assessments of high-risk patients
• Collaborate with primary care providers, discharge planners, and support staff to implement individualized care plans
• Coordinate care transitions across settings
• Address both clinical and non-clinical needs (e.g., transportation, housing support)
• Educate patients and families on disease management, medications, and care goals
• Document thoroughly using HomeCare HomeBase (HCHB) EMR
• Ensure regulatory and compliance standards are met
• Promote wellness and prevent avoidable readmissions or complications
• Act as a patient advocate and maintain strict confidentiality

✅ Requirements

• Active RN license in a compact or multi-state jurisdiction
• AHA BLS certification
• Home Health Case Management experience required
• Proficiency with OASIS and HomeCare HomeBase (HCHB) required
• Strong wound care knowledge
• Reliable transportation, valid driver’s license, and proof of auto insurance
• Current TB test and competency documentation
• Case Management certification preferred
• Associate’s or Bachelor’s Degree in Nursing

Mileage reimbursed at $0.45 per mile

Coverage area includes Charlottesville to Staunton

Must complete and submit skills checklist (including wound, HIPAA, TB, infection control, OSHA, and CPR training)

Scrub colors not specified; professional attire expected

Field-based, onsite role – not remote

Flexible weekday schedule, but 6 points/day required
