We’re seeking experienced and compassionate Registered Nurses (RNs) to join our growing team, providing both high-acuity and low-acuity patient care in Alcalde, New Mexico.
Patient requires 24/7 care, we have day and night shift options availableIdeal for nurses looking for stable, full-shift work in a home settingIncludes more complex clinical care and monitoringOpportunity to build strong, long-term relationships with patients and families

Requirements:Active RN license in the state of New MexicoReliable transportationAbility to work independently

Responsibilities:

• Execute physician prescribed plans of care.
• Complete compliant documentation of the care provided in the system of record.
• Manage and administer prescribed medication, treatment, and therapies.
• Conduct patient assessments, coordination of care.
• Perform various activities associated with daily living.
• Monitor vitals and GI intake and output.
• Assess skin integrity and administer wound care.
• Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
• Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
• Provide education and training to the patient, their family, and/or the Home Health Aide.
• Work with patients to be done one-on-one in the patients home to provide for a comfortable environment.
• Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
• Perform other personal care services as necessary to meet the patient’s needs.

RN Hourly Rate: Up to $50 an hour, based on experience.LPN Hourly Rate: Up to $35 an hour, based on experience.
