At Home Harmony is seeking a compassionate and detail-oriented Medical Assistant to join our growing in-home medical practice. This role is central to ensuring seamless coordination of patient care by supporting our Advanced Practice Providers (APPs) and Physicians through visit preparation, telephonic patient check-ins, and real-time clinical support. Following a successful probationary period, this position will offer the flexibility of hybrid work (work from home and in office) for eligible duties.

Key Responsibilities:
  • Conduct telephonic patient check-ins prior to scheduled visits to verify demographics, medications, allergies, and recent health changes.
  • Support APPs and MDs by ensuring all pre-visit documentation is complete and uploaded into the EMR (Athena).
  • Coordinate patient scheduling, appointment reminders, and follow-up calls.
  • Review hospital discharges and communicate key updates to the provider team.
  • Assist with incoming patient calls, messages, and coordination between departments (pharmacy, personal care, GUIDE program, etc.).
  • Document accurately within EMR and maintain compliance with HIPAA and internal privacy policies.
  • Track labs, imaging, and referral requests; ensure timely communication of results to providers and patients.
  • Participate in daily huddles and interdisciplinary team meetings.
  • Support population health initiatives and care coordination workflows.
  • Perform administrative or clinical support tasks as assigned by leadership.
  • May be expected or asked to visit patients in their homes to collect clinical documents.
  • May be asked to visit patient homes to set up Remote Patient Monitoring (RPM) devices, including blood pressure monitors or scales.
  • May be asked to conduct home visits for clinical services such as blood draws, vaccine administration, and/or urine sample collection.
  • Candidates should be comfortable entering patient homes and performing clinical duties in a home-based environment.
  • Certified or Registered Medical Assistant preferred (CMA, RMA, CCMA).
  • Minimum 12 years of clinical or telehealth experience in a medical practice or home-based care setting.
  • Strong computer skills and familiarity with EMRs (Athena experience preferred).
  • Excellent communication and customer service skills.
  • Ability to work independently and manage multiple priorities.
  • Reliable and professional with strong attention to detail.
  • Need to pass either driving history screen or have reliable transportation with proof of insurance.
Qualifications, Schedule & Work Environment:
  • Monday - Friday, 8:00- 4:30
  • Office-based during initial training and probationary period.
  • Eligible for remote work (partial WFH) upon completion of probation, based on performance and departmental needs.
Compensation & Benefits:
  • Competitive hourly rate (commensurate with experience) - $20-$23 Hour
