Job Description

JOB SUMMARY:

The Clinical Specialist is a mental health professional reporting directly to the Division VP. This primarily clinical and consultative role provides services across all supported housing programs (both single- and scatter site) within the cluster. While not having formal supervisory responsibilities, the VP may delegate tasks to staff at the Program Director level and below in relevant Supported Housing (SH) programs.

ESSENTIAL JOB FUNCTIONS:

Assist Program Directors and case management staff in providing quality clinical and crisis management services through consultation and training. Consult on challenging clinical cases involving high-risk consumers, co-morbid medical conditions, or compliance issues, maintaining a caseload while primary services are managed by case management staff. Organize and facilitate internal case conferences, working with staff to design and implement person-centered, evidence-based interventions to resolve issues, motivate consumers, and determine case dispositions. Attend external case conferences on behalf of the Cluster; participate in discharge planning decisions involving SH programs, inpatient settings, detox/rehab programs, etc. Conduct risk assessments in supported housing, ensure completion, inform relevant staff about high-risk consumers, and oversee follow-up actions. Provide staff training (e.g., independent living skills, wellness activities) and co-facilitate groups as needed. Oversee clinical quality and service planning integrity; train staff in these areas. Participate actively in Clinical-High Risk Meetings and all meetings related to supported housing. Assist in determining readiness for transition to supported housing, identifying and addressing skills training needs. Participate in discharge planning and case conferences. Conduct intake assessments for new admissions, including ADL evaluations. Review documentation such as Support Plans, progress notes, and risk assessments. Perform administrative and operational tasks as assigned. Perform other related duties as required.

ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES:

Ability to coordinate staff activities and work as part of a multidisciplinary team. Effective problem-solving skills. Respectful and caring interaction with consumers, families, and staff, with cultural sensitivity. Skill in developing, evaluating, implementing, and modifying case management plans within deadlines and productivity standards. Proficiency in accurate and timely documentation. Effective communication skills with staff, consumers, families, and the public. Understanding and adherence to relevant laws, rules, and policies. Knowledge of human behavior, mental health assessment, and behavioral disorders. Understanding of group dynamics, societal influences, and cultural factors. Knowledge of service provision principles, needs assessment, quality standards, and consumer satisfaction evaluation. Familiarity with applicable laws, regulations, and agency policies. Ability to conduct field work and travel as needed.

QUALIFICATIONS AND EXPERIENCE:

MSW, MHC, or Master's in Psychology with at least one year of direct service experience with individuals with mental disabilities, and one year of clinical supervision or consultation experience. PREFERRED EDUCATION AND/OR EXPERIENCE: LMSW or LMHC licensure preferred; experience in case management for mental health and substance use disorders; bilingual in Spanish preferred but not required.
